We'll always hand out hard copies of our forms to you, but you can also download any TEAM form right here. Please take a moment to find the correct one(s).
For overnight field trips, you'll need the Extended Field Trip Parent Waiver Form .
For day trips, you'll need the Parent Day Trip Permission Waiver Form .
If your child takes medication that needs to be administered during the day or at night while on a field trip, please select the appropriate medication form listed in the attachments below. Please allow time to get the doctor's signature.
If you are driving on a field trip with your personal car, please fill out the appropriate form. Be aware that you'll need proof of insurance and a copy of your driver's license.
